Da Form 5690 Reserve Components Career counselor I is a civilian employee who assists in the development of career plans for Soldiers undergoing active duty and reservist service. The RC career counselor also assists in identifying Soldiers for RC affiliation and extension and prepares the DA Form 3340 for signatures. In addition, the RC career counselor coordinates the referrals of eligible Soldiers who decline reenlistment, and the payments of accrued leave.
The application includes copies of DD Form 4 and DA Form 3540, which certify the applicant meets the requirements for USAR service and the method of fulfilling those requirements, and DA Form 5435, which certifies that the applicant understands the Selected Reserve MGIB (10 USC Chapter 1606).
