What is a DA FORM 7015?
The DA Form 7015 is the form used to nominate civilians for the Superior Civilian Service Medal. The medal is awarded to civilians who have made significant contributions to the Department of the Army. To be eligible for the award, civilians must have served at least 10 years in a senior position in the Army. The form must be filled out by current or former supervisors, and it requires an extensive amount of information about the nominee, including his or her accomplishments.
