A DA Form 7249 is a military document that documents an individual’s service in the U.S. Army Reserve. The document indicates the applicant’s status as a member of the Selected Reserve and acknowledges the service requirements and methods of fulfillment. It includes the date of the last physical examination as 16 August 1994, the physical profile as 11111, no physical limitations, the applicant’s signature, and the dates of drills from November 1994 to April 1995. The DA Form 7249-R must be signed by the soldier in order for the reservation to be complete.
Where Can I Find a DA Form 7249 ?
When a tax return is accepted, the IRS prepares a Public Inspection File containing the taxpayer’s redacted Form 7249 and sanitized AOIC Masterfile Screen transcript.
