What is a DA Form 4591?

A DA Form 4591-Retention Counseling record is a document that is used to record information regarding the DA Retention Program. It is prepared by a Non-Commissioned Officer for all soldiers who are subject to indefinite reenlistment contracts. The records must be written in pencil, pen, or digitally. In addition, the Enlisted Record Brief should be attached to the DA Form 4591. The entries on the form must be verified by a servicing career counselor.

The purpose of a Retention Counseling Record is to determine whether an individual is eligible for re-enlistment or not. In addition, it is used to determine if an individual is eligible for a waiver or extension of their enlistment. The records should also include information on how to apply for an extension.

Where Can I Find a DA Form 4591?
