What is a DA Form 7764-5

The Army Musician Proficiency Assessment (AMPA) is a mandatory document that provides the minimum scores required for accessions to and completion of the Noncommissioned Officer Education System (NOES). This form can also be used to evaluate a musician’s musical skills.

Before a Soldier can be considered for ACASP, he must complete a proficiency training course. After successful completion, he can be awarded the MOS. Then, he must complete proficiency training in accordance with the option he selected. After completing the AIT, the RC unit commander may award him the MOS.
